What are your office hours?

Fairfax location: Monday through Friday 8:30am to 5pm
Fair Oaks location: Tuesday 1pm - 5pm, Wednesday 9am - 1pm, Thursday and Friday 9am - 5pm

How do I make an appointment?

Call 703-698-8525
When requesting an appointment, the scheduler will ask for the following information:
• Your name
• Your Date of birth
• Home and alternate phone numbers
• Your primary doctor or referring physician
• Your insurance or HMO information

Are there any forms that I can fill out at home prior to my appointment?

Yes! We STRONGLY encourage you to fill out all the online forms prior to coming for appointment as to expedite your visit.

Do I need a referral?

If you participate in an HMO or Point of Service (POS) Health plan, we request that you obtain a referral or authorization from your primary doctor prior to your appointment and make certain you bring it with you to the appointment.

What do I need to bring as a new patient?

• Primary care physician contact information
• Referral (if required)
• Insurance card(s)
• All medications that you are taking (including prescription and non-prescription)
• Medical records (such as catheterization reports, stress test results, echocardiograms)
• Picture ID

What time should I come for my appointment?

Please plan on arriving early (20 minutes) to be registered and complete patient forms.  For your convenience, all necessary forms are available online which you may complete at home and bring with you. (Highly Recommended

How do I obtain prescription refills?

We ask that patients request all necessary cardiology prescriptions while seeing our providers at the office visit.
You can call our office and speak to our nursing staff for refills, but due to the extra time required for calling in/mailing prescriptions, you will be charged $10 for all the prescriptions if you have been seen within the past 1 month.

What happens if I need to reschedule/cancel the appointment?

We ask that you call as soon as you need to reschedule and request at least 24 hours notice.  Because of our desire to be as available as possible to patients, if you miss an appointment without notifying our staff at least 24 hours prior to the appointment, you will receive one warning and then be charged for any future “no shows” (the “no show” charge depends on the type of appointment or type of service/test missed).
